The Centers for Medicare & Medicaid Services (CMS) recently announced significant changes to how primary care providers will be paid starting in 2025. APCM introduces three new payment levels based on patient complexity and social needs, offering monthly payments ranging from approximately $15 to $110 per patient. Unlike current chronic care management codes, APCM eliminates burdensome time-tracking requirements while expanding eligibility to more patients. Key features include the ability to provide 24/7 access to care, comprehensive care management, and enhanced communication opportunities – all without specific time thresholds.
