Remote Patient Monitoring (RPM) allows providers to bring consistent and reliable care into the lives of their patients by collecting vital health data remotely. RPM programs utilize digital devices such as blood pressure cuffs, weight scales, pulse oximeters, and more. The collected data is securely transmitted to a central platform, enabling providers to monitor chronic conditions like diabetes, heart failure, and COPD.

We take a comprehensive approach to remote patient monitoring and create custom workflows to meet each customer’s needs, from enrollment to logistics management, patient education to billing. Patients gain access to health condition monitoring, alerts and reminders and intervention from connected devices.
Our experience strongly validates that RPM programs are more effective when combined with a patient engagement strategy and complementary programs, such as Chronic Care Management (CCM). When RPM and CCM are offered together, patients receive telephonic coaching calls from staff that have access to the same condition monitoring data and patient logged vitals that providers see within the EHR. Armed with health history as well as patients’ real-time trending conditions, we see patients are more engaged when their care is more responsive.
Medicare Billable Codes
We enable provider reimbursement for the following RPM CPT codes:
- 99453: Initial patient set-up and education, billed once per patient.
- 99454: For 16 days of device connection and transmission of physiological data.
- 99457: The first 20 minutes of clinical monitoring and support services, per calendar month.
- 99458: An add-on code to 99457 for additional clinical monitoring time per calendar month, billed in 20 minute increments.
- G0511: For Rural Health Clinics or Federally Qualified Health Centers (RHC or FQHC) only, 20 minutes or more of general care management provided by clinical staff. G0511 incorporates multiple codes to keep costs down, covering services for Remote Patient Monitoring and many more.
