Annual Wellness Visits (AWV) are yearly preventive healthcare appointments covered by Medicare. These visits focus on gathering vital information, assessing health risks, and creating personalized plans to optimize patient well-being. The AWV provides an opportunity for physicians to improve the quality of care, assist in patient engagement, and optimize payment opportunities.
Preferably prior to the AWV, but at the very least during it, the health risk assessment (HRA) must be completed by the beneficiary or healthcare provider. The HRA includes questions that assess a variety of health topics such as fall prevention, cognitive impairment, and advance care planning. The results of this questionnaire help guides the face-to-face consultation, hence why many primary care providers enlist the support of care management partners to prepare the HRA in advance, or use purpose-built software to streamline the process.
